Service Coordination
What is Service Coordination?
The Service Coordination Program is for families who are involved with multi-systems due to the needs of their children. It is a process to help the family identify and coordinate the services they are currently receiving and services they may not know about to provide support in maintaining the child in his/her home in a safe and healthy environment. It will help families and professionals work together with one coordinated plan.
Who is Service Coordination for?
- Youth from birth to 21.
- Youth involved in multiple systems across our community, such as the courts, Job and Family Services, Board of Developmental Disabilities, Special School services and/or mental health services.
- Youth at risk of a change of placement due to behaviors, significant mental health needs, Job and Family Services involvement and/or court involvement.
